After your patient registration is complete you will be able to log onto your secure patient portal and fill out the General and Nutrition intake forms and consents. These intake forms are located in your patient portal under the Questionnaires tab. Please complete the forms under the headings: New Patients and Consents and Nutrition Forms. These need to be completed at least 24 hours prior to your appointment with Dr. Salazar and Megan M. Rouse respectively.
